I used get a dialog appear with older versions, recently updated to this one. Now there is a little icon. I click it, but nothing appears. I tried deleting all the files and reinstalling but no luck. How do I get the dialog to appear?

This is quite strange...
Can you successfully use any other versions ? e.g. v1022-04...
Replacing all of the files shouldn't be necessary, BUT if you do it'd be everything in the Octane sub-folder and its sub-folders... and the Octane_loader.rb for completeness - though you'd get nothing if that file were broken!
If you are up to it, please close Sketchup, then 'Run...' regedit and go to HKEY_CURRENT_USER\Software\Google\SketchUp7... then find the entries for WebDialog_Octane and WebDialog_Octane_FolderBrowser and delete them both - these set the size of the dialog windows, which might have somehow become corrupted if as you say the dialog isn't a sensible size any more.
Then reinstall v1022-05 and try opening a SKP again...
Let me know how it goes...

Sounds like something is adrift in the dialog making process.
I suspect that you have perhaps not got all of the required files into the ../Plugins//Octane/ folder - e.g. Octane.html which is needed to make the dialog = the error-messages about that...
To ensure a 'pure' installation you must 'extract' all of the files/sub-folders in the zip-file into a temporary folder on your Desktop, then move them all into the ../Plugins/ folder keeping all of the files in their correct sub-folders.
If you find that there are previously installed files in the wrong place please delete them first [compare names/file suffix etc].
When you 'move' the newer files you must confirm to overwrite any existing files with the newer ones.
The README file in the zip [and/or Help file] clearly lists which files should be in which folders/sub-folders.
A common error is to 'unzip' everything directly into the 'Plugins' folder without keeping the required sub-folder structure that is set in the zip-file [some zip-applications are somewhat lax in this regard - also dragging-and-dropping files from the zip-file into ../Plugins/ is a recipe for disaster because the sub-folders can get ignored] - as a result there will be many files in the wrong place and then you'll get error-messages when some files aren't found and loaded as needed.
Note that Version 1022-06 is about to be launched... so I suggest that you upgrade to that too...
