I've seen that "crash once, re-launch, runs fine" behavior, too. I'm guessing it's writing a file it needs to be present as it initialized the plugin the first time you launch. The second time, the required file is there, so it's happy.I bought a new card (GTX 1060), because it seemed very hardware related. In order to get it running I needed to update to OSX 10.12.5 and install the new drivers for the GPU.
So in the end I updated everything: OSX, GPU driver, CUDA driver and Octane VR. Im still on c4d R18.041.
When I restarted I had a crash right away but after that one crash it seemed to run properly; I had it rendering for 4 hours, aborted the render (which always used to cause c4d to crash) and it didn't crash one time. So fingers crossed tonight and tomorrow; hopefully it won't crash and it's finally solved!

So there are endless combinations of c4d software + CUDA drivers + OctaneVR versions, but if it's so random, isn't it just hardware?
How can I possibly try all these software combinations?! That would take me forever...

You have to apply the update to a clean install of C4D, or at least one older than the update you are running.I downloaded the file you mention, and if I go Help> Manual Installation and select the update file, it says there is no update. Probably because I'm on R18.041?
