GPU failed?

I'm using a Quadro P1000, and as shown in the image, it says the gpu is failed. Any one can help me on that? Thx!!!

I'm using 4.01, and it seems I can't run OctaneBench, either. It says failed to allocate device memory. Not sure how to fix this. Thx!

the Octane Bench works. But the octane standalone stiil says Quadro P1000 failed. I opened out-of-core, not sure if that matters

Hi,
4.01 is too old, and cannot work with latest Nvidia drivers.
Please, upgrade to 4.05 and report back:
