3. The background image comes back, it's just that EVERY time I try to animate it disappears again.

I've tried a couple of scenes with no luck repeating your issue. If you can, can you PM me your scene. Or you can wait till tomorrow and try the new release I'll be posting.

Are there any key frames on the background? Try removing them if there are.
Does the problem happen if you move the timeline from within Carrara?
Please try the following:
Find the octane_log_flags.txt file in the OctaneRender extension directory.
Enable the apiRender flag (remove the leading '-').
Open your scene, the ORVP, and the log window.
!!! Clear the contents of the log window.
From within Carrara single step the scene ONCE!
The log window should now be full of node change diagnostics. Please send it to me. If it is long please zip it and send it as an attachment.
Don't forget to disable the apiRender flag when you are done...
