This problem has been going on for a long time and afaik Octane is the only render engine affected by this:
render instances of render instances won't render!
This is a BIG problem when importing STEP files with instances enabled. You get lots of instances which are part of some machinery and if this machinery is instanced again, it won't render. With scenes that contain 10.000 objects or more, there is no way to find all conflicting render instances - and if you eventually do, you double or triple your poly count.
Is there any way to fix this?

It's not added because of complexity of implementing this. It may be easier in any other renderer but our plugin system is not yet allowing to add that. I've tried that to do a few times but it always caused a lot of problems and gone back.
