Why my geometry tears apart when applying displacement

I have this issue of tearing objects when using displacement

Can anyone help me on this how to correct the attached image.

It works fine on plain but when using on deformed objects, my geometry tears apart

Anyone help will be deeply appreciated

Try to switch from Texture Displacement type to Vertex Displacement, and increase the subdivision:
