Achieved with displacements mapping and a Mix Material node for the skin vs the flesh. If the flesh is sufficiently thin, I use an opacity map to cut the rest of it out making it "punch through" to the other side. The pumpkin model is hollowed out by a similar amount that the pumpkin is displaced by.
