You've hit all the nails in terms of art: form, composition, mood etc and to be honest it's an absolutely fabulous piece: bravo!
One question I have: the detail work such as the wrinkles and skin pores etc - is that geometry or normal maps/bump maps?

I'm not sure I understand the purpose of it. Could you elaborate? I tend to steer away from the daylight illumination model as I find it offers very little ambient depth. I did several iterations of the original layout with daylight without much success. The detail work on her skin is a combination of all those three things. I feed several normal maps and bump maps on different shader layers. The head model being fed to the engine is fairly low resolution, 1.2 million verts (see below.) The normals maps and bump maps (derived from normals and mixed with procedural textures) do the rest of the job.

Please check the Live DB. tonysculptor already generously shared his skin shader with all of us. This is the one I used as a base for my work. You would be better off with the original version available there.
Also, an advice for the future, try to avoid quoting entire messages and associated pictures.
