Yes, IES settings look right.
Octane is just acting as a normal camera does - so I think your solution lies in photography rather than Octane settings. If you took this shoot with a camera - what lighting would you need to get a clean image? Certainly a normal camera will produce a grainy image with the light levels you have circled - there is no light in there to produce a clean result. It's exactly the same in Octane.

Just one think on the IES lights - I think you need to set a gamma of 1, not 2.2.
