everything remains the same.
I have placed the plugin in another folder and still does not load.
I installed lightwave about 20 times and I have repeated the option to load the plugin again.
I checked microsoft visual 2012.
since they can not do more

Well, I am lost
One more test ... can you please put the octane.dll, octane.dat and tbb.dll files in the same folder than the plugin .p file, and try again?
Anyway, if you have skype, feel free to contact with me to chat about this issue (juanjgon71) ... can be something easy to fix, but I am not sure what could be the problem.

I have formatted disk and reinstalled WINDOWS

...yep if you change your windows version and/or the disk C: for example ...(i mean if you change some from your hardware set up) it will be happen what jcastillo wrote...
buts thats more a hardware checksum error or so...i can fix it any time with a short step to the bios and play with the UEFI settings...after the next start then all its fine...
