Do you see any error messages in the log? And did you do a clean install?

have You tried to check power connectors of those cards? just to unplug & plug again, switch them.. etc - I have a feeling it's somewhat PSU related..-I'd say turn some stress test & see with GPU Z if all cards are running smoothly =)

1. Take off all of gpus out of your motherboard.
2. Insert just one titan into every pcie slot and test it.
3. Insert two titans (1,2), (1,3), (2,3) into all possible pairs of pcie slots (blue1, black1), (blue1, blue2), ...
4. Insert three titans in all possible combinations.

For every setup run clean install of nvidia driver (if anything fails).

5. If you can - test three titans with another PSU - one pcie power lane may be dead in your psu.

I did a clean install for each driver hoping one of the old drivers was the issue but that didn't help. I did notice that when I do a PCIE render test in GPU-Z it will only stress the 1st card, no matter which card I select in GPU-Z to do the render test. So that was odd ...

Is this setup new? I mean was it working fine anytime before?

Using 3 cards is new, I've used 2 cards for the past couple years without a problem.

I thought about another reason - take off your cpu and make sure you have all its pins straight. Another words - your cpu and the socket shall not be damaged.

Thanks smicah, im not sure what setup your referencing with the image though between the x'ed out spots and the circles around the other two connectors?

If you insert a gpu into the lowest pcie slot you'll not be able to connect usb cables (crossed). If you still encounter your problem - make sure your cpu socket and its pins are not bent.
