Why my geometry tears apart when applying displacement

I have this issue of tearing objects when using displacement :(

Can anyone help me on this how to correct the attached image.

It works fine on plain but when using on deformed objects, my geometry tears apart :(

Anyone help will be deeply appreciated :)

Try to switch from Texture Displacement type to Vertex Displacement, and increase the subdivision:
