There are two same passes saved when using OpenExr multilayer format – Combined.Combined and RenderLayer.Combined
Sometimes an empty Z pass is also saved even when not enabled.
Two same combined passes are saved.

I have had this happen in past as well. It seems random. At least I have not been able to identify the parameters it occurs with. Another anomaly observed is sometimes the z pass is rendered improperly as being very large (i.e. an enlarged portion of just part of the scene) and not as a solid color but rather as a series of lines or dots.
