Okay, so the first set of peeks all at 100% are without Octane's AS.
Turn on AS you start to see the GPU use less. So it looks like it is not re-using cores to finish the render.

I see so many posts about it not running at 100%, well it is AS. It's still faster but not efficient on core usage.

Yeah, RNDR is a really poor setup. Really the best system would be for the host plug-ins to see RNDR as GPUs or be able to send first the scene data and then just update the scene frame by frame rather than packing a full scene.
Personally, I'd rather see the RNDR GPUs as virtual GPUs on your own ...

The problem is the speed the external cards send & receive data. The bottleneck is the cable. It's only a x1
Also, don't use those cards for tone mapping or denoising, this will slow things down too.

I just rendered a scene with 180 frames following Juanjo's recommendations to turn off multi-threading and I haven't had the same problems.

I will continue to render other scenes and hope this doesn't happen again.

As for the possibility of removing the NGons that is out of the question, because ...

I have seen vanishing objects before, as I said check for ngons they can create all sorts of problems.
I had this job with lots of machines controlled by both morphs & bones. These were imported cad objects, some would just vanish from the render, turned out to be the ngons I had not removed or ...

Lewis is right, this use to work. Tested in Octane 2020.2.4.0.
Confirmed: If you hide the instancing object in this case a cube the instances still render in older versions.
