Any idea where this is for 3ds Max plugin, or which version?

Searching on these forums comes up with nothing for max but plenty for Lightwave, Houdini etc. Which is odd...

If your license is v4, then and it will require an annual maintenance upgrade in order to use Octane 2019.
You would consider to:
Upgrade your legacy V4 perpetual licenses to V2020.x perpetual licenses:

Annual maintenance includes the following:
- Access to Octane 2019 and any future versions of Octane throughout the duration of your subscription

or purchase Octane Enterprise Maintenance legacy V4 (or V4 AA) perpetual:
12 Month Extension 199 €
24 Month Extension 349 €
Octane Enterprise Maintenance allows you to get continuous access to all future updates and new features beyond OctaneRender V4.

, for 'only' E199/year?

Presumably

'continuous access to all future updates and new features'

includes V2020.x?

Erm, right.

So options are;

200 for V4 (which I have anyway?!?)

500 for V2020 (for any RTX, NV Link, bloom threshold functions)

or

20/month to rent V2020.
