Is it possible to use the CADItem and push it to Octane with out having to convert it to mesh item?

In theory, if you enable Preferences->Octane->Use Modo Render Cache, this should work. I haven't tried it myself though. And the Modo Render Cache is still not 100% robust, so use with caution.

Sadly no luck with the Render Cache

If you send me a sample scene, I will take a closer look.
