Procedural to get spots like speckled paint?

This is actually a Poser material for a nail set, you can see they used a node "3D Textures -> Spots." It's on the specular channel for that glitter nail look.

I want to use this in Octane for the nails, how can I do it without resorting to creating a 2D image for it?

Try a Mix in the Diffuse pin, with a Noise node in the Amount.
