You've hit all the nails in terms of art: form, composition, mood etc and to be honest it's an absolutely fabulous piece: bravo!

One question I have: the detail work such as the wrinkles and skin pores etc - is that geometry or normal maps/bump maps?

Your shader was a fantastic help. I had been scratching my head for weeks without much success before that. I modified it quite heavily and included a couple extra layers to simulate the otjize (the red paste Himba people cover their skin and hair with) layer. I always marvel at how robust the shader ends up being under most lighting conditions.

I did several iterations of the original layout with daylight without much success. I ended up switching over to a homemade HDR pano and a couple reflectors to push more light from below.

Do you mind sharing your Skin Shader? I have been desperate for a good skinshader for the 3DS Max plugin ~ one that includes a texture map like yours

The detail work on her skin is a combination of all those three things. I feed several normal maps and bump maps on different shader layers. The head model being fed to the engine is fairly low resolution, 1.2 million verts (see below.) The normals maps and bump maps (derived from normals and mixed with procedural textures) do the rest of the job.

Please check the Live DB. tonysculptor already generously shared his skin shader with all of us. This is the one I used as a base for my work. You would be better off with the original version available there.
Also, an advice for the future, try to avoid quoting entire messages and associated pictures.
