plese try to update the GPU drivers at this version:

If you cannot install , you need to apply first the latest Security Update.

I just started to have the same exact issue about 30 minutes ago.
(A note for everyone, do not update past MacOS sierra 10.12.6 if you have 3 GPU's... IT WILL NOT WORK. This is the absolute latest version I can have with support of 3 GPU's)
A. MacOS Sierra Version 10.12.6 (16G1618) Latest security update
B. 3 (Nvidia GP100)
C. 128 GB RAM
D. Nvidia Driver 378.05.05.25F11 (Latest)
E. OctaneRender Standalone version, 4.00
F. OctaneRender plugin version C4D 4.00
G. Host application version, including build number C4D R19.068 (Build RB245443)
I: Octane Log:

CUDA error 999 on device 2: unknown error

Open C4D, with live viewer already docked in a window, and before I even start a scene, RENDER FAILURE message pops up in the window. LV will not work. Also Octane Benchmark 4.00b gives the same Cuda 999 error on all three cards. (I have tried each one) Octane bench 3.06.2 works fine however. Please advise since your above recommendation about updating to the latest driver does not work based on the fact that I have the latest drivers and operating system combination that works with 3 GPU's.

If you find the solution I'm interested. Because even with the update of the latest drivers I always have the same error

These are the step I have tried with no success..
Uninstall all octane software (stand alone and plugin) and reinstall. (used app cleaner software)
Re installed the cude drivers
tried reverting back to octane 3 (which now is giving me the same error as 4)
The only thing I have no tried is uninstalling the cuDNN libraries. I did install them over again but I do not know how to remove cuDNN manually. Nothing works.
I have a large project I am now behind on and now NONE of the octane plugins are working. PLEASE HELP. Standalone does not work either and neither does the octane bench 4. Only octane bench 3 works. Again, I am in need of some pretty desperate help!

Already solve exact this problem. Try anything, with no success. Only Octanebench works, but no Octane Standalone/plugins.
Solution is only update mac os to high sierra (10.13.6 with last security updates) and reinstall nvidia/cuda drivers to last versions.
Since update to High Sierra, Octane works fine, but still hangs my Mac while using AI Denoiser.

Hi,
if your Mac hangs while denoising, check the available system RAM (not VRAM0, if not enough, you can have this kind of issues.
Try to reduce the resolution to see if it works with less system RAM consumption.
